Percutaneous Endoscopic Lumbar Discectomy for Far-Migrated Disc Herniation through Two Working Channels.

چکیده
UNLABELLED The technique of percutaneous endoscopic lumbar discetomy (PELD) in the transforaminal approach has evolved over the years due to the advances in endoscopic photology and instrumentation and become the most popular technique for lumbar disc herniation. Although PELD offers many advantages, the indications of PELD are limited mostly to non-migrated or low-migrated disc herniation. It is very difficult for PELD in the transforaminal approach to remove the highly migrated disc fragment successfully due to the anatomic barrier. Nowadays, with the advances of instruments and technique, it might be possible for PELD in the transforaminal approach to remove these high-grade migrated disc fragments. The purpose of this study was to describe a technique to effectively treat highly migrated disc herniation via 2 working channels. KEY WORDS Percutaneous endoscopic lumbar discectomy, far-migrated disc herniation, working channels.

Surgical Outcome of Two-Level Transforaminal Percutaneous Endoscopic Lumbar Discectomy for Far-Migrated Disc Herniation
Objective. To describe the two-level percutaneous endoscopic lumbar discectomy (PELD) technique in transforaminal approach for highly migrated disc herniation and investigate its clinical outcomes. Methods. A total of 22 consecutive patients with highly migrated lumbar disc herniation were enrolled for the study from June 2012 to February 2014. Results. There were 12 males and 10 females, with ...

Purpose To evaluate the efficacy of suprapedicular circumferential opening technique (SCOT) of percutaneous endoscopic transforaminal lumbar discectomy (PETLD) for high grade inferiorly migrated lumbar disc herniation. Material and Methods Eighteen consecutive patients who presented with back and leg pain with a single-level high grade inferiorly migrated lumbar disc herniation were included....
متن کاملA Modified Approach of Percutaneous Endoscopic Lumbar Discectomy (PELD) for Far Lateral Disc Herniation at L5-S1 with Foot Drop
Foraminal or extraforaminal Far Lateral Disc Herniations (FLDH) extending into or beyond the foraminal zone have been recognized as between 7-12% of all lumbosacral disc herniations. Conventional posterior laminectomy may not provide good access to a herniation that lies far lateral to the lateral margin of the pedicle. Use of the endoscopic technique through a percutaneous approach to treat su...
متن کاملExtreme Lateral Endoscopic Transforaminal Lumbar Discectomy for Lumbar Disc Herniation in Young Patients
Objective: The herniated lumbar disc (HLD) in young patients is increasing. The endoscopic discectomy has been used as a treatment option, but it is limited in the management of large extruded or migrated disc herniations. Recently, with an application of extreme lateral endoscopic lumbar discectomy (ELELD), it is possible to obtain access to epidural space directly and to remove herniated disc...
متن کاملPercutaneous Endoscopic Lumbar discectomy for Foraminal to Superior Migrated Disc at L5- S1 level using the Contralateral Interlaminar Approach: A Technical Case Report
Objective: In cases of foraminal superior migration of L5-S1 HNP, it is very difficult to access by rigid endoscopic procedure. Thus, we attempted a contralateral interlaminar approach to expose the exiting nerve root and contralateral foramen to remove the symptomatic disc and preserve the functional structures. Methods: Between January 2013 and January 2014, five patients who received the per...
